New Patient Coordinator

As part of our personalized care experience, our New Patient Coordinator will reach out to you shortly after you contact us. Their goal is to make your onboarding process simple and stress-free. They will assist in scheduling your first visit, collect and verify your insurance information, and answer any questions you may have about getting started.

You will also receive an email with a secure link to complete your electronic intake forms, which include sections on family history, social history, surgical history, medical history, current concerns, and consent forms. Completing these forms in advance allows us to understand your background thoroughly and ensures we are fully prepared to provide the highest quality care from the moment you arrive.

When You Arrive

When you arrive at Dynamic Health Clinic, our welcoming front desk team will be ready and expecting you. Please remember to bring a valid photo ID and your insurance cards with you. We understand how important it is to listen to your concerns and fully understand your health history, which is why we take the time to make sure you feel heard, supported, and valued from the very start.
